📣 Attention 5th Grade Families!
Our annual California Healthy Kids Survey is coming up soon for all 5th-grade students.
Permission slips are coming home today. Please review the form, sign it, and return it to your child’s teacher as soon as possible.
To make it a little more fun, we are adding two incentives:
🎁 Family Raffle: Every parent or guardian who returns a signed permission slip will be entered into a raffle for a $50 Amazon Gift Card.
🍩 Class Challenge: The 5th grade class with the most returned permission slips will receive donuts for the entire class!
The California Healthy Kids Survey gives students an opportunity to share important information about their school experience and helps us better understand how we can continue to support student safety, belonging, wellness, and success.
