Dear Parents and Guardians,
During November and December, our 5th-grade students will take the California Healthy Kids Survey (CHKS).
This brief, anonymous survey asks about school safety, climate, and student well-being. The results help our district improve programs and support for students.
You can look at the survey before it is given by asking at our main office or visiting: [https://www.slusd.us/families-community/california-healthy-kids-survey]
- Grade 5: A permission slip was sent home in October 2026.
- Parents/guardians must sign and return the slip for students to take the survey.
Attached a copy of the Permission Slips for your reference.
Please sign the complete the permission slips and return it to your child's teacher.
